Tendonitis Q&A

What is tendonitis?

Tendonitis is the inflammation of tendons — tough connective tissues that attach muscles to bones. While the foot and ankle contain several tendons, the Achilles tendon is the one most commonly affected.

The Achilles tendon (also known as the heel cord) is the largest tendon in the body. It runs along the back of your lower leg and connects the calf muscle to the heel bone.

Tendonitis typically results from overuse or repetitive strain. A sudden increase in physical activity or prolonged stress can cause microtears in the tendon fibers, leading to inflammation and pain. Without adequate rest, continued stress can prevent proper healing and worsen the condition.

What are the risk factors for tendonitis?

Tendonitis is common among athletes and individuals whose jobs involve repetitive foot or ankle movements. It also frequently affects “weekend warriors” — people who engage in intense physical activity without regular conditioning.

Flat feet or excessive pronation (inward rolling of the foot) can also increase your risk, as they place additional strain on the tendons during walking or running.

What symptoms does tendonitis cause?

Common symptoms of tendonitis include:

  • Ongoing or recurring pain
  • Aching or soreness
  • Stiffness
  • Tenderness to the touch

Pain may be more noticeable after periods of inactivity, improve slightly with movement, and then worsen again with prolonged activity.

If left untreated, tendonitis may progress to tendinosis — a degenerative condition where the tendon begins to break down. Signs of tendinosis can include tendon thickening or the formation of nodules (small lumps) at the injury site.

At Pinnacle Foot & Ankle Clinics, your provider will examine your foot and ankle to assess the range of motion and identify any signs of tendon injury. Diagnostic imaging such as X-rays, ultrasound, or MRI may be used to evaluate the severity of the damage.

How is tendonitis treated?

Treatment for tendonitis or tendinosis depends on the duration and severity of the condition. Your personalized treatment plan may include:

  • Rest and activity modification
  • Cold therapy (ice packs)
  • Anti-inflammatory medications
  • Physical therapy
  • Stretching and strengthening exercises
  • Custom-fitted orthotics (supportive shoe inserts)
  • Steroid injections
  • Laser therapy

Most cases of tendonitis respond well to conservative treatment. However, if the condition is severe or unresponsive to non-surgical methods, surgical intervention may be recommended to repair the damaged tendon.
